In order to make registration an efficient process,
we ask that you have the following documents and information ready:

Birth Certificate
Social Security Card
Immunization Records—Complete and up-to-date for age
Proof of residence (such as a utility bill, rent receipt, etc.)
Last school’s name and address and report card if coming from a school not in Conroe ISD.
Withdrawal form if enrolled during current school year
Custody Papers (signed by a judge, if child is not living with both natural parents)
Additional information for enrollment forms:

Names and phone numbers for physician and/or hospital
